To ensure an accurate claim is processed you must ensure that we have access to all of the relevant information at a Practice, Patient and Claim level including:
- Office Address
- Office phone number
- Tax ID and NPI
- Full name of the Dentist that provided the service.
- License Number of the Dentist that provided the service.
- Patient name and DOB
- If not that patient, the Subscriber name and DOB
- Patient ID
- In case a patient has multiple plans with the same provider, their Group Number
- Date of service
- Procedure(s) provided
- Total amount being billed to the insurance company as listed on the claim
- Attachments included with the claim such as: x-rays, narratives, perio chart or any other documents submitted with the claim.
